Co-producing a Theory of Change (ToC) is a collaborative process where stakeholders work together to define and visualize the pathways leading from current activities to desired long-term impacts. This participatory approach ensures that the ToC reflects the collective insights, experiences, and aspirations of all involved, fostering a shared understanding and commitment to the process.

The benefits of co-producing a ToC include ensuring inclusivity, as it incorporates diverse perspectives and experiences, leading to a more holistic and robust framework. Additionally, it provides clarity and alignment by offering a clear roadmap for achieving desired outcomes and aligning activities and resources with long-term goals. This process also facilitates strategic planning by identifying key steps, assumptions, and potential risks, and establishes a framework for monitoring progress and evaluating success, thereby enhancing accountability.

The first step in co-producing a Theory of Change (ToC) is to gather a diverse group of stakeholders, including community members, practitioners, and decision-makers. The objective of this step is to ensure that all relevant perspectives and expertise are represented, fostering a comprehensive and inclusive process. By inviting participants who have a vested interest in the project or initiative, we ensure that the ToC is informed by those who are directly impacted and those who play key roles in its implementation.

**Supporting Iterative Learning**: A ToC encourages ongoing learning and adaptation. It helps in defining methods for continuous monitoring and formative evaluation, allowing the organization to make real-time adjustments based on what is being learned.&#x20;

For example, the first iteration of our Theory of Change relied on a lot of untested assumptions and hypotheses on how dimensions of our service enabled us to achieve one of our core missions of allowing care and support to exist in greater abundance. The recent pilot in Commons-based care was an opportunity to put some of these assumptions to the test and develop a more granular vision of the pathways for achieving this goal. The most recent iteration of our ToC maps out some of the outcomes and impacts of promoting community ownership and greater service integration with local care and support networks.&#x20;
