What we're aiming for and why
A Theory of Change begins by identifying the outcomes and impacts an organisation is working toward. These outcomes are the anchor points of any meaningful evaluation framework: they help us stay focused on what truly matters.
Our Theory of Change maps out the goals we seek to achieve across the care ecosystem, from the well-being of care receivers to the strength of community networks. These outcomes reflect a vision for social care that is relational, inclusive, and driven by the people who give and receive care.
Well-being and relationships.
Our ToC recognises that individual well-being is inseparable from the quality of relationships people have: with each other, their support teams, and their wider community.
Outcomes here focus on emotional, social, and relational well-being as core parts of care, not just extras.
Personal Growth & Individual Agency
We aim to create environments where all members of the co-op - care receivers, workers, family, friends, and volunteers - feel secure, supported, and free to grow.
Many of our outcomes reflect this focus on autonomy, dignity, and empowerment.
Co-production
Our evaluation is fundamentally about measuring how well people work together.
Outcomes relating to co-production look at the quality of collaboration, shared decision-making, and whether people feel genuinely included in designing their care.
Community Connection & Belonging
We value care that connects people: to one another and to their wider communities.
Outcomes in this area capture how well our service builds inclusion, fosters mutual support, and strengthens community ties.
Quality of lives
Rather than focusing solely on an individual’s quality of life, our ToC takes a broader view: how well are we supporting everyone in the ecosystem - care workers, recipients, families, and volunteers - to live better, more connected lives?
