# Outcomes

A Theory of Change begins by identifying the outcomes and impacts an organisation is working toward. These outcomes are the anchor points of any meaningful evaluation framework: they help us stay focused on what truly matters.

Our Theory of Change maps out the goals we seek to achieve across the care ecosystem, from the well-being of care receivers to the strength of community networks. These outcomes reflect a vision for social care that is relational, inclusive, and driven by the people who give and receive care.

This theme reflects our belief that care is about relationships, not transactions. </p><p></p><p>The outcomes here reflect the trust, safety, and compassion that define a good care experience.</p><h4></h4></td></tr><tr><td><strong>Co-production</strong></td><td><p>Our evaluation is fundamentally about measuring how well people work together. </p><p></p><p>Outcomes relating to co-production look at the quality of collaboration, shared decision-making, and whether people feel genuinely included in designing their care.</p></td></tr><tr><td><strong>Community Connection &#x26; Belonging</strong></td><td><p>We value care that connects people: to one another and to their wider communities.</p><p></p><p>Outcomes in this area capture how well our service builds inclusion, fosters mutual support, and strengthens community ties.</p></td></tr><tr><td><strong>Quality of lives</strong></td><td>Rather than focusing solely on an individual’s quality of life, our ToC takes a broader view: how well are we supporting everyone in the ecosystem - care workers, recipients, families, and volunteers - to live better, more connected lives?</td></tr></tbody></table>
