# Theory of Change

Equal Care Co-op’s model was designed from the ground up to hold true to our founding principles:

🌱 Enabling care and support to flourish in abundance

At the heart of our approach is our interactive **Theory of Change**. It’s a tool to help make sense of how the different parts of our work connect  and how they lead to the outcomes we want to see. It guides our planning, helps us track impact and supports us to stay aligned with our values.

This isn’t a one-off or a static diagram. Our Theory of Change is something we revisit, reflect on, and evolve. For example, the pilot of a commons-based care model in Hackney led to a clearer understanding of some of the indicators and outputs that relate to a "commons-based approach" and the tool has grown in response.

Whether you're looking to work with Equal Care or working on your own care project elsewhere, we hope this Theory of Change helps you think through your targeted impacts. It’s there to be adapted, built on and improved.
